Southside Community Services’ Croatian Community Care Program provides assistance to the frail aged, younger people with a disability and their Carers from Croatian Communities to access Home and Community Care (HACC) and other services.
The aim of the Croatian Community Care Program is to assist people from Croatian Communities who might be experiencing cultural or linguistic barriers to access a variety of services, including those offered by HACC.
The program offers support for people who are aged or have a disability, to maintain their independence and quality of life for as long as possible in their own homes.
CARE COORDINATION:
The Croatian Community Care Program has two bilingual Case Managers to assist clients in accessing HACC and other services such as:

Advocacy
Providing information
Home and Hospital visits
Residential Respite
Allied Health Services
Aged Care Facilities
CROATIAN DAY GROUP:
The Croatian Day Group is designed specifically for people who are originally from Croatian communities. Group activities are held every second Wednesday and include English lessons and excursions into the community.
COST:
There is a small charge for some services. Those who are financially disadvantaged will not have their services refused or withdrawn for that reason.

Hrvatski Program Kućne i Društvene njege pruža pomoć boležljivim starijim osobama s umjerenim do teškim smetnjama, mlađim osobama sa smetnjama, te njihovim njegovateljima iz srednje– europskih zajednica da žive samostalno u vlastitim domovima i da sudjeluju u životu svojih društvenih zajednica.

CIJENA USLUGA:
Očekuje se da klijenti sami novčano doprinesu pokrivanju troškova nekih usluga, ovisno o njihovim primanjima, no usluge neće biti uskraćene nikomu ukoliko nisu u mogućnosti platiti.
Hrvatski Program Kućne i Društvene Njege zapošljava dvije djelatnice koje govore Hrvatski i Engleski jezik.
